Life and Accidental Death & Dismemberment (AD&D) Insurances

Basic Life and AD&D insurances are provided by the company at no cost to benefits eligible Full-time and Part-time associates per the following schedule:

  Basic Life* Basic AD&D*
Full-time associates $25,000 $30,000
Part-time associates $8,500 $10,500

Combined maximum for basic & optional coverage is $4.5 million for life and $1.5 million for AD&D) and associate paid dependent coverages**.

Optional Life Insurance, Dependent Life Insurance and Optional Accidental Death and Dismemberment (AD&D) Insurance

 

Optional Associate Life
1x salary to 5x salary ($4,500,000 maximum)
  • Dependent Life Insurance: Pays a benefit to the associate if his/her covered dependent(s) die.

  • Optional AD&D: Associate only or associate plus dependent
Optional Associate AD&D
1x salary to 6x salary ($1,500,000 maximum)
  • Optional Family / Dependent AD&D Insurance: Pays a benefit to the associate if his/her covered dependent(s) die as a result of a covered accident or if his/her covered dependent(s) lose a limb, sight or hearing as a result of an accident.

*Merchandisers, Field Coordinators, Revision Leads, leased employees, independent contractors and certain union employees are not eligible for the programs listed here. However, Field Coordinators and Revision Leads are eligible for the optional programs.

**If the associate and the associate's spouse both work for the company and are both benefits eligible, the associate may not be covered as a dependent for Optional Life and/or Optional AD&D and only one may choose coverage for dependent children.
