Basic Life and AD&D insurances are provided by the company at no cost to benefits eligible Full-time and Part-time associates per the following schedule:

*Merchandisers, Field Coordinators, Revision Leads, leased employees, independent contractors and certain union employees are not eligible for the programs listed here. However, Field Coordinators and Revision Leads are eligible for the optional programs.
**If the associate and the associate's spouse both work for the company and are both benefits eligible, the associate may not be covered as a dependent for Optional Life and/or Optional AD&D and only one may choose coverage for dependent children.
